We are sending home repeat forms for children in reception onwards for parents to opt in/out to having their children receive their flu vaccine on 8th October at school. Thank you to those who have already sent their forms in before summer.
It is very important we receive your forms back so we can collate ready for the school nurse to collect next week, even if you don’t wish for your child to take part. Please ensure you sign the form clearly.
